Outline and application method of specific infertility treatment expenses subsidy in Japan

“I want to work on fertility treatment, but the explanations about subsidies and application methods are mostly in Japanese. It’s a very private matter, so no one can ask me.”

I have no children, but I definitely wanted to help my friend. So, in this article, I will explain in detail the outline and application method of the specific fertility treatment subsidy in English. What is the specific infertility treatment expenses subsidy?

Money is a concern for people who say, “I think it’s time to think about in vitro fertilization(IVF) …”.  Also, there are many people who say, “I was surprised that it cost me unexpectedly when I started to get pregnant!” Many infertility treatments are not covered by Japanese health insurance and tend to be expensive.  The government’s “Specified Infertility Treatment Expenses Subsidy Program” provides financial support for such pregnant couples. The targets of this support are “in vitro fertilization” and “intracytoplasmic sperm injection”.  Please note that timing methods, ovulation induction methods, artificial insemination, etc. are not covered by the subsidy.

Conditions for those who can receive the subsidy

To receive the grant, you must meet the conditions.  What are the conditions?  Let’s check it together. People who meet all of the following four conditions can apply for and receive the grant.

A person who is judged by a doctor to have no or extremely low chance of becoming pregnant with treatment methods other than “specific infertility treatment”. Only those who have been diagnosed by a doctor can receive the subsidy.

Persons who receive specific fertility treatment at a medical institution designated by the local government.

This applies to married couples who are legally married.  However, common-law couples are also eligible if they meet the requirements.

The age of the wife on the first medical examination day when she started the specific fertility treatment must be 42 years or younger.  Unfortunately, it does not apply to people over the age of 43.

How much can I receive the grant?

If you meet the conditions, you will be wondering how much you can receive the subsidy.  The upper limit of the subsidy is set by the treatment stage (treatment content).

Depending on the treatment stage (treatment content), you can receive a subsidy of up to 100,000 to 300,000 yen per treatment.

Example 1)
If treatment for treatment stage A is performed and it costs 280,000 yen, the cost of 280,000 yen will be paid.
Example 2)
If treatment for treatment stage A is performed and it costs 350,000 yen, the maximum amount of subsidy of 300,000 yen will be paid.

What is one treatment?  How to count the number of treatments

The “one-time treatment” that is eligible for the subsidy is the process from the start of medication to prepare for egg collection to one in vitro fertilization or microinsemination.  Frozen embryo transfer using fertilized embryos made by in vitro fertilization or intracytoplasmic sperm injection performed in the past is also a single treatment. The end date of one treatment is the date of confirmation of pregnancy (whether pregnant or not) or the date of discontinuation of treatment at the discretion of the doctor.

By the way, the number of grants is determined by the age of the wife.

  • Under 39 years old … Up to 6 times per child
  • 40-42 years old … up to 3 times per child
  • 43 years old and over … Not eligible for subsidy

In addition, if you give birth after receiving a grant or if you have a stillbirth after the 12th week of pregnancy, you can reset the number of grants you have received so far. 
* Only those who wish to reset the number of grants can apply.  You do not have to apply.  If you reset, the number of remaining grants may decrease depending on the age at the start of treatment after the reset, so check with the person in charge at the application counter in advance.

  • If you need a copy of yourself, please make a copy and submit the original.
  • You can download it from the web page of each local government.

Certificate of consultation for specific infertility treatment cost subsidy project
(特定不妊治療費助成事業受診等証明書 / とくてい ふにんちりょうひ じょせいじぎょう じゅしんとう しょうめいしょ)

  • Fill in by the designated medical institution.
  • One sheet is required for each treatment.
  • If you need a copy of yourself, please make a copy and submit the original.
  • You can download it from the web page of each local government.

Certificate of Resident’s (My number(individual number) is not listed on the certificate of resident’s)

  • Issued within 3 months from the application date.
  • You can pick it up at the window of the local government.
    If you have an Individual Number Card, you can also issue it at a convenience store.
  • It costs several hundred yen to issue.

  • You can pick it up at the window of the local government.

After completing all the necessary documents, mail them to the designated address of each municipality. By the way, in the case of Tokyo, the destination is as follows.

In addition, there is a deadline for applying for grants.  Apply within the year to which the day when the “one treatment” is completed belongs.  The fiscal year refers to the one year from April 1st to March 31st of the following year.  For example, if the “one treatment” is completed on February 20, 2021, the postmark of March 31, 2021 must be stamped.  If you post it to the mailbox on the night of March 31, it will be collected on the morning of April 1 the next day, and the postmark stamped at the post office will be “April 1”.  Please note that in this case, the application will be overdue and will not be accepted.

Notification of results and receipt of grants

It seems that it takes 3-4 months to notify the result after applying.  Approximately one month after the result notification, the grant will be transferred to the bank account you specified.
